I use an online registration system for new clients.  Once we have scheduled an intake session, I will send you an email with a link to my online registration, where you will be asked to complete a brief intake questionnaire; and review several forms (similar to what you sign at a doctor's appointment):

  1. Treatment Policy and Disclosure Statement (required for all therapists in Washington State)
  2. Notice of Privacy Practices (also required)
  3. Health Insurance Disclosure Agreement (required if you will be using your health insurance)
  4. Cancellation Policy (24 hours)
  5. Fee Schedule

Authorization to release information:

To speak with another healthcare provider, teacher, lawyer, family member, etc., you (or the parent/guardian, if the client is a child) must sign an authorization form. This can be downloaded here:
