I use an online registration system for new clients. Once we have scheduled an intake session, I will send you an email with a link to my online registration, where you will be asked to complete a brief intake questionnaire; and review several forms (similar to what you sign at a doctor's appointment):
- Treatment Policy and Disclosure Statement (required for all therapists in Washington State)
- Notice of Privacy Practices (also required)
- Health Insurance Disclosure Agreement (required if you will be using your health insurance)
- Cancellation Policy (24 hours)
- Fee Schedule
